Eligibility


  1. You must reside in one of the following Nebraska counties: Adams, Boone, Buffalo, Butler, Cass, Chase, Clay, Colfax, Custer, Dawson, Dawes, Dodge, Fillmore, Franklin, Gage, Garden, Hall, Hamilton, Harlan, Holt, Howard, Jefferson, Johnson, Kearney, Knox, Lancaster, Lincoln, Madison, Merrick, Nance, Nemaha, Nuckolls, Otoe, Pawnee, Phelps, Platte, Polk, Richardson, Saline, Saunders, Seward, Sherman, Thayer, Ord, Webster, York. Residents of other counties in Nebraska may still apply based on financial need, and grants will be considered based on available funds.​
  2. You must meet the Foundation’s FINANCIAL NEED ELIGIBILITY CRITERIA.​
  3. Be under the care of a physician with a treatment regimen in place at the time of application.

Grant Restrictions


  • Patient must be a U.S. citizen or legal U.S. resident.
  • The Foundation may ask at any time for further documentation to support a patient’s eligibility, including after any grant has been extended. Any falsification of an application is fraudulent and subject to potential criminal penalties and civil damages. 
  • ​The Foundation will make no referrals or recommendations regarding specific providers, practitioners, suppliers, products, or plans. 
  • Patients will not be informed of the identity of specific donors. 
  • The determination of a patient’s financial qualification for assistance will be based solely on his or her financial need, without considering the identity of any of his or her health care providers, practitioners, suppliers, products, or insurance plan; the identity of any referring party; or the identity of any donor that may have contributed for the support of the patient’s condition. 
  • ​Assistance will be based upon a reasonable, verifiable, and uniform measure of financial need that will be applied in a consistent manner. 
  • Patients are free at any time to switch providers, practitioners, suppliers, or products without affecting their continued eligibility for financial assistance. 
  • Medicare beneficiaries are free to switch insurance plans when permitted by the Medicare program, without affecting their eligibility for assistance.

At the Heartland Cancer Foundation (HCF), all financially eligible applicants receive consideration for a grant without regard to race, color, religion, gender, gender identity or expression, sexual orientation, national origin, genetics, disability, age, or veteran status. This commitment ensures that all individuals affected by cancer have access to the resources they need to receive the best possible care and support.

Additionally, HCF has taken steps to ensure that our website is accessible for the 20% of the population living with disabilities, such as those with visual, auditory, motor, and cognitive impairments. By implementing accessiBe, a WCAG and ADA compliant tool, all users can perceive, understand, navigate, and interact with the website and apply for support. 

Finally, by committing to compliance with medical care acts and privacy laws, HCF is demonstrating its commitment to treating all individuals with respect and dignity. This commitment to compliance helps to ensure that all individuals are protected from discrimination and that their medical information is kept confidential.
